To avoid bleeding out, replenish your blood near campfires, and to prepare for it, always keep food and water on you.
It deals additional damage to the blood gauge, so it doesn’t target HP but the blood bar which heals more slowly, making it good because your opponent can’t spit to heal their blood or anything.
Failing to survive in The Depths and perishing will result in you being sent to the Fragments of Self. This is basically the afterlife, the final memories of the character. There is not much you can do here.
